Job Title: Clinical Pharmacist, Medication Reconciliation & Transition of Care Specialist

Reports to: Manager – Clinical Client Care Services

No. of Positions: 1

Location: Markham, Head Office

Position Type: Contract

Salary: Salary, commensurate with experiences

Key Responsibilities

Responsible for building and maintaining healthy customer relations with designated Long Term Care facilities, in conjunction with the pharmacy team.

Providing professional functions including, but not limited to:

  • Conduct the formal process of medication reconciliation in accordance with MPGL policies and procedures on all MPGL clients pending admission to LTC, retirement living, etc. in a timely manner with a goal to improve patient outcome
  • Provide comprehensive pharmaceutical care in collaboration with the interdisciplinary team and make recommendations to the clinicians
  • Work with the healthcare team to develop strategies to optimize resident care in transition and improve outcome

Administrative duties such as, but not limited to:

  • Produce quality metrics related to clients and medications in transitions
  • Support departmental initiatives to deliver quality patient care in accordance with MPGL, Department of Clinical Innovations and Quality policies and procedures

Medication Management such as, but not limited to:

  • Liaising closely with the dispensing pharmacy, manager and staff to ensure optimal pharmacy service to each facility


  • Excellent written/verbal communication, interpersonal and presentation skills
  • Proven analytical and problem-solving experience
  • Ability to organize and prioritize work, and perform multiple tasks plus analytical and problem-solving ability.
  • Self-motivated and results orientated
  • Able to work effectively within the organization to establish relationships, build trust and influence appropriately to drive performance through learning


  • Bachelor’s Degree in Pharmacy required, Pharm D and/or BCGP preferred
  • Current pharmacist license in good standing with the Ontario College of Pharmacists
  • Experience in a Long Term Care pharmacy environment is required; previous consulting experience is preferred
  • Proficient in the use of PDF, Microsoft Office applications such as Excel, Word, Outlook
  • Clear and recent Vulnerable Sector Police Check
