CLINICAL CONSULTANT PHARMACISTS
Our team of clinical consultant pharmacists is a unique group of professionals. They are the front line interface with each long term care home we service. With the full support of head office, they apply their expertise to resolving the issues and concerns of the residents and staff.
Medical Pharmacies' clinical consultants make a difference to the well-being and quality of life to residents across Ontario - one person at a time.
Each member of our team of over 30 clinical consultant pharmacists participates in psychogeriatrics, palliative care, pain control and disease management. All are members of the Canadian Society of Consultant Pharmacists (CSCP) and the American Society of Consultant Pharmacists (ASCP).

Many of our clinical consultant pharmacists pursue advanced studies to further demonstrate their dedication to the care of seniors. A growing number have achieved their CGP (Certified Geriatric Pharmacist) as well as Doctorates of Pharmacy and Masters in Gerontology.
Medical Pharmacies supports our clinical consultant pharmacists in their ongoing education and involvement in pharmacy specializations.
Research is essential for the development of advancements in pharmacy care for seniors. Our pharmacists have participated in a number of studies including a national study on medication use in long term care homes and drug usage evaluations.
Many of our clinical consultant pharmacists are active members of community support groups related to the elderly including the Parkinson’s Society and the Alzheimer’s Society of Ontario.
In recognition of their excellence in the practice of geriatric pharmacy care, a number of our consultants have been honoured with the CSCP Eldercare Award and the Fellow of the American Society of Consultant Pharmacists.
Clinical Consultant Services
Medical Pharmacies provides support beyond our in-pharmacy clinical pharmacy services. Our clinical consultant pharmacists participate as integral members of each long term care home’s multi-disciplinary healthcare team.
They focus on individuals through comprehensive medication reviews for each resident to identify, assess and solve medication-related problems with the objective of enhancing their quality of life. Each clinical consultant pharmacist is supported by a team of over 30 others. The team is an information resource on processes and specializations.

Clinical consultant services provided include:
- Regularly scheduled clinical consultant visits timed conveniently to meet your clinical and administrative needs
- Visit reports - Clinical consultants provide a report to administration on their activities and recommendations from each of their visits to each home
- Assessment and monitoring of medication therapy with a focus on the appropriate use of medication and minimizing the risk of adverse effects
- Resident and family counselling, in individual or group settings, to address questions and provide additional education around medication usage
- Participation in professional advisory meetings bringing current pharmacy and drug related information that is relevant to the individual home and of interest to the professional team
- Statistical reporting on drug usage to facilitate internal reporting, analysis and external benchmarking
- Continuing education - Current information on pharmacy practices and guidelines is provided through creative, flexible, educational programs
- Quality assurance audits - In-depth audits are conducted to ensure compliance with all relevant legislation, regulations and standards
- Drug destruction methods to meet ministry standard which are efficient and environmentally sensitive
- Preparation for infectious disease outbreaks including individualized dosing of appropriate therapies plus ready-to-use order forms and MAR sheets in the home
- Preparation for accreditation by contributing expertise and experience to team meetings and surveyor interviews
- Orientation and training on pharmacy systems conducted one-on-one, in group settings and aided by our own Medical Pharmacies training videos
- Facility specific programs addressing specific disease state issues, educational needs, etc. can be developed where necessary to address specific issues in a home
